HSAGP is registered with IMMTRAC. To keep an electronic file with our database, we enter all of our students' immunization information with the registry. We ask that parents sign a consent form before they are entered. If for some reason you feel that your student should not be added to the registry, then a decline notice needs to be signed and submitted.
Although it is highly recommended that you vaccinate your children, we understand there can be other factors or personal beliefs in which a child my not have immunizations. If you chose not to give your child immunizations, there is a waiver that must be requested through the Department of Health to be in compliance and to be accepted to any school in the state of Texas.
